Continuation of Benefits

Upon leaving the University of Texas System, you may continue your current health coverage. To do so, you will need to:

1. Contact your local UT benefit office and request information regarding COBRA benefits. Your health benefits will remain the same under COBRA. The rates, however, will change. These rates will be given to you by your benefit office, along with the appropriate forms to enroll.
2. If you wish to elect COBRA, please fill out the COBRA form and send it directly to Humana at the following address:

3. Along with the COBRA enrollment form, please send in the first month premium. After receiving your enrollment form, Humana will process your application and send you an invoice for the amount owed each month.
