Do you need additional HumanaDental ID cards?

To request additional identification cards online:

• Log into HumanaDental.com with your User ID and Password (first time users will need to create a User ID and password)
• Select the Plans & Coverage tab
• Select Details under the Dental PPO Plan Summary
• Select the Member Details tab
• Click on the Order ID Card link
• A pop-up will appear asking: Would you like to have a new Humana ID card sent to you?
• Select Yes
• You will get a confirmation pop-up indicating that your ID card request was successfully submitted. You can expect to receive your new ID card(s) within the next 7-10 business day. Thank you for visiting us online.

To request additional identification cards by phone:

• Call Motorola Rewards Administration Center at 800-421-3973
• Press 2 (To hear specific member information)
• Press 1 (To hear information about your 2008 dental benefits. You will be transferred to HumanaDental)
• When prompted, say the word OTHER
• Indicate that you are a member
• Choose ID cards
• Enter your 9-digit HumanaDental ID (located on the front of your dental ID card), date of birth, and your zip code.
• The system will automatically request the ID cards for you
