Dental Provider Network Search

You may begin your provider search by selecting HumanaDental. Once there,
  • enter your Zip Code under “Search by Coverage & Network”
  • under “Select Your Network”, select “PPO/Traditional Preferred”
  • on the next page check “I agree to the Terms of Use” and choose “Search by address” or “Search by state and county”
  • finalize your search by selecting from the list of specialties or locating a specific dentist or group/facility
The HumanaDental website includes additional offices of the provider along with appropriate phone numbers for the office location (and a fax number, if available). The information also includes your estimated distance to the provider’s office and a link to map the exact location and provide driving directions.

Nomination Status

If your dental provider is currently not participating with HumanaDental you may nominate your provider by calling 1-800-432-4804. The HumanaDental customer service center is open from 8 a.m. to 6 p.m. (in your time zone) Monday through Friday or accessing the “Refer a Dentist” site.

To access the “Refer a Dentist” website simply follow these steps:
  • Go to HumanaDental.com
  • On the bottom right-hand corner under “Tools & Resources”, click on “Refer a dentist” · Complete the requested information and hit “submit”
HumanaDental will be accepting dental provider nominations from Motorola participants through December 31, 2009. Monthly updates are made regarding dental provider nominations. Check the status of a dental provider nomination.

From the time of the nomination to final conclusion, either acceptance or declination, it is approximately 7 weeks. The dental provider will be contacted shortly after HumanaDental receives the referral. In about one week, to allow the dental provider an opportunity to review the contract information, a follow-up contact is made by HumanaDental. Calls are made weekly until a final decision is reached. Once the completed application is received by HumanaDental, the credentialing process and activation of their contract takes approximately 10-14 business days.

Nomination of a dental provider does not guarantee HumanaDental network participation. It is an individual business decision by the dental provider. Some dental providers may not be willing to accept discounted rates. It is also important to note that a small percentage, generally less than one (1) percent, may apply but not meet the HumanaDental credentialing standards.
